Event Final Report

Thank you for choosing to report your Paddle America Club / Division / Affiliate Organization event online.  If you have not already downloaded the excel document "Final Event Report e-form" to submit below, you can open it here, fill it out, save it, and submit below. 

Upon completion of this form, you will be directed to the eStore where you can pay any applicable Event Membership Fees.  If you do not wish to pay them online, please include the appropriate payment when you mail in the original signed waivers.

Download the Event Final Report Form and submit below. Please list names, addresses, email addresses and ACA numbers of all participants. If applicable, enclose a copy of your activity/event results. Failure to file this e-form in a timely manner may jeopardize future insurance coverage. Original, signed waivers of non-ACA members must be sent to the ACA separately.
