Registration and Requirements

To request insurance through the ACA General Liability Insurance Policy, Affiliates must:

• Download and complete the ACA Affiliate Insurance Request Form and submit by email, fax or US Mail to the ACA Insurance Coordinator
• Any sanctioning or additional fees must accompany the insurance application in order to be processed. You may call in a credit card number if you feel uncomfortable sending the information via mail or fax.
  • Insurance Sanctioning fee (per event) is $25
  • Insurance applications received less than 10 days prior to the class are subject to an additional $25 "rush fee"
  • Third Party (additional insured) insurance is available for an additional $20 per additional insured
  • The $25 sanctioning fee, $25 rush fee and $20 additional insured fees are non-refundable.

If your event / activity is insured through the ACA General Liability Insurance Policy, all participants must:

• Be a current member of the ACA or
• Pay the $5 event membership fee or
• Join the ACA at the time of the program
• Sign an approved ACA Waiver and Release of Liability form prior to participation in your program.
  • ACA Adult Waiver & Release of Liability (Spanish) (with membership form)
  • ACA Minor Waiver & Release of Liability (with membership form) (Under Age 18 At The Time of Registration)

Upon completion of the event / activity (within 30 days of the last day of the insured activity), be sure to submit:

• All fees collected at the event
• Original copies of the signed ACA approved waivers for all participants
• Final report form (see below)
