Online Form-ACA Assessment Course Registration

For ACA Instructors and / or Program Coordinators, please fill out the following to submit your ACA Assessment Course Registration Form (with or without Insurance coverage) to the Safety Education & Instruction Department online.

Current CPR/1st Aid is required and must be submitted with each registration. You will have the opportunity to upload a scanned copy below, or fax them in when you are done submitting the form and fees.

As the ACA Instructor in charge of this event, I hereby agree that the course/workshop will be conducted in accordance with all ACA requirements, risk management, and all other rules, guidelines and conditions established by the ACA. I have read and fully understand all sanctioning requirements established by the ACA. I have personally inspected the event site(s) and I attest to the fact that such site(s) are appropriate for use in this event and free of undue hazards.
If you are requesting ACA Insurance for this course please check Yes and fill out the below fields. If not, please check No, input NA in PAC fields, then submit.
