This form is to be submitted along with all associated fees within 7 days of the course completion.  This form is only necessary for courses that have been insured.  You can use the EZ Skills Course Report form for uninsured courses.
  • Only one course can be reported per report form
  • Click here to access a blank roster to fill out, save and upload
  • You will have the opportunity to upload the Course Roster at the end of this form
  • Payment must be received by the Insurance Department before this report will be considered closed

If this course was taught by an IT/ITE, update credit can be given to assisting instructors. To issue this update credit, please note below and include the level of update.
If this course was insured by ACA Insurance, please upload scanned, unaltered copies of the ACA Waiver and Release of Liability forms for each participant and instructor.

Please note Non-ACA Member participants require a $5.00 Event Membership fee only if the course is covered by ACA Insurance.
